Here are a few details to assist you with your first visit.
WHAT TO BRING:
- Please bring attire that allows access to the injured body part, along with clean indoor running shoes
- Insurance information if you would like to set up direct billing
- Alberta Health Care Number
ADMISSION PROCESS:
Upon your arrival, you will receive an initial assessment form, which will provide us with detailed information regarding your injury and general health. Please give yourself 5-10 minutes to complete the form prior to your appointment.
INITIAL ASSESSMENT APPOINTMENT:
Your appointment will last 45-60 minutes and will include an injury assessment as well as treatment. Together, you and your physio will establish your treatment goals and a plan to achieve them.
PAYMENT:
We accept cash, debit, Visa, and MasterCard.
We also direct bill for the following providers (if your plan allows):
Blue Cross, Desjardins, GWL, Green Shield, Industrial Alliance, Johnson, Johnson Industrial, Johnston Group, Maximum Benefit, Manulife, Medavie Blue Cross (DND, DVA, RCMP), and Sunlife.
Private Motor Vehicle Accident insurance is also accepted.
Please note the following:
We do not have a contract with WCB and thus cannot treat individuals who have workplace injuries.
We do not have a contract with Alberta Health Services, therefore do not have funding for post-operative or post-fracture cases.
